What is hyperhidrosis?

Hyperhidrosis is a condition that causes a person to sweat more than normal, even when it is not hot and they are not exercising. It may affect the underarms, hands, soles, face or a combination of these areas.

Primary hyperhidrosis often begins in childhood or adolescence. Depending on severity, it may affect daily activities, confidence when greeting others, use of tools, school, work and psychosocial wellbeing.

In some patients, excessive sweating causes stress and significant difficulty during social interactions. It can also affect work; for example, patients with severe hand sweating may struggle to hold instruments, write or manipulate objects because they slip.

For this reason, each case should be evaluated individually. It is important to measure severity, affected areas, previous treatments and the real impact on daily life.

Treatment options

Antiperspirants, botulinum toxin or surgery

After evaluation, treatment options are determined according to severity, affected area, prior treatments and patient expectations. Some patients may be candidates for stronger antiperspirants.

Others may benefit from botulinum toxin injections, for example Botox™ for underarms, which can significantly reduce sweating for several months. In moderate to severe cases with important psychosocial impact and after trying several lines of treatment, definitive surgical therapy may be considered.

Thoracic surgery

Minimally invasive thoracic sympathectomy

Minimally invasive thoracic sympathectomy may be considered in selected patients with moderate to severe hyperhidrosis, especially when there is significant psychosocial impact and insufficient response to previous treatments.

The decision should be made after discussing expected benefits, risks, alternatives, the possibility of compensatory sweating and postoperative care. Surgery is not for every patient; it requires individual evaluation.

Underarm Botox for axillary hyperhidrosis

For patients with excessive underarm sweating, botulinum toxin/Botox™ may be a non-surgical option to significantly reduce sweating for several months.

The indication is defined after reviewing severity, quality-of-life impact, previous treatments, and HDSS/HidroQoL questionnaires. Not every patient is a candidate, so individualized evaluation is important.
